CGHS Card Renewal
Central Government Health Scheme plastic card is issued with a validity of 5 years. Every employee who has claimed a CGHS card for himself and family members must renew their respective card once the renewal period has come. Failure to do so will lead your CGHS card to go invalid and won’t help any health benefits.
Application form for renewal of CGHS card will be accepted by Additional Director of CGHS from the city of employee belonging. To avail government benefits by wellness centers, employees need to track their benefits and the validity period of the card.
Through this card, employees and their family members can get their most health checked in the top hospitals, and any health issues and daily prescription and medicines can be claimed under the benefits of the CGHS Card.
Application form for CGHS Plastic card need to be submitted with a current photograph, Copy of Payslip, Address Proof, Employee ID card, and exact details of family members can be submitted, if their valid has been expired too.
Office of Administrative of Additional Director will issue a new CGHS Plastic card with a valid of 5 years with the same beneficiary name added. The Government of India has provided a benefit to the employee to apply for their new card three months before their expiry.
This procedure of renewal should be followed by serving employee and for an employee who has done their service need to change proofs, where the Pensioner CGHS Cardholder needs to submit LPC or PPO proof along with newel application form, So make sure you get Blue Color card if you’re a serving employee, Green color Card for Pensioners & Freedom Fighters, Red Color Card for Member of Parliament and Yellow Color card for Autonomous Bodies & Journalist.
